📋 Description
• Coordinate care for members with chronic medical and behavioral health conditions • Conduct comprehensive assessments and address social determinants of health • Develop and implement individualized care plans • Document care management activity in electronic health record • Collaborate with interdisciplinary care team members • Advocating for necessary services and ensuring optimal health outcomes
🎯 Requirements
• Active and unrestricted Registered Nurse (RN) licensure in Pennsylvania OR Compact Registered Nurse (RN) licensure in state of residence • Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams • Access to a private, dedicated space to conduct work effectively • 3+ years of nursing experience • 2+ years of case management, discharge planning and/or home healthcare coordination experience
