📋 Description

• Facilitate delivery of appropriate benefits and healthcare information to determine benefit eligibility • Promote wellness activities • Develop, implement, and support health strategies, tactics, policies, and programs • Support delivery of benefits, overall member wellness, and successful and timely return to work • Perform assessment, planning, facilitation, care coordination, evaluation, and advocacy • Coordinate options and services addressing individuals’ and families’ comprehensive health needs • Communicate with members, providers, and community organizations • Use available resources to promote quality and cost-effective outcomes • Collaborate with Community Care teams in a member-centric, team-delivered, community-based care management model

🎯 Requirements

• RN Compact license required • Minimum 5+ years clinical practical experience with Medicare members; preference for diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac experience • Minimum 2+ years of discharge planning and/or home health care coordination experience • Minimum 2+ years Case Management Experience • Excellent analytical and problem-solving skills • Effective communications, organizational, and interpersonal skills • Ability to work independently • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient and effective computer skills, including navigating multiple systems and keyboarding • Willing and able to obtain multi-state RN licenses if needed; company will provide • Bilingual preferred • Certified Case Manager preferred • Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) preferred, but not required • Associate degree or Nursing Diploma required • Bachelor’s degree preferred
