📋 Description

• Serve as a bridge between Aetna members and the healthcare system through outreach and education • Conduct home visits with members with chronic conditions • Reinforce care plans and adjust tasks to fit members’ culture, language, and religion • Educate healthcare professionals and providers on social determinants of health • Build and maintain relationships with members, providers, nurse case managers, agencies, supervisors, and office staff • Reduce cultural and socioeconomic barriers between clients and institutions • Assist with care plan implementation and member education during in-home visits • Develop care management strategies and provide linkages to health and social services • Help members identify socioeconomic issues and develop health/social management plans and goals • Identify gaps in community care systems and assist members with community services, appointments, transportation vendors, and program applications • Support medication adherence by reviewing medication lists and helping obtain refills • Teach disease self-management, including nutrition and symptom tracking • Accompany members to appointments as needed • Maintain member records in electronic health record systems and complete reports and program documentation • Travel extensively to client homes, community locations, agencies, and outreach destinations

🎯 Requirements

• Must reside in one of the following parishes: Orleans, St. Bernard, Jefferson, Plaquemines • Ability to travel 70% of the time • Must have access to reliable transportation • 3-5 years' relevant work experience • 1-2 years’ experience providing clinical support services • 1 year of community outreach experience and knowledge of LA community resources • 1 year’s experience working with clients with clinical needs (physical and/or behavioral health) • Demonstrated proficiency with a personal computer, keyboard navigation, and MS Office Suite applications, including MS Teams, Outlook, Word, Excel, and Electronic Health Record Systems • Flexibility to work outside of core business hours Monday-Friday, 8am-5pm • Ability to work 2 days weekly until 9pm local time • Community Health Worker Certification or equivalent training and experience preferred • Bilingual speaking skills in Spanish or French is a plus
