📋 Description

• Utilizes a collaborative process of assessment, planning, implementation and evaluation. • Engages, educates, and promotes/influences members decisions related to achieving and maintaining optimal health status. • Facilitates effective health care decision-making. • Improves the quality of care. • Improves member satisfaction with their health care plan. • Facilitates appropriate use of health care resources. • Provides up-to-date health care information and assistance by clarifying available options for health problems. • Promotes healthy lifestyles to help lower personal health risks.

🎯 Requirements

• Active unrestricted RN compact state licensure. • 3+ years of clinical experience required, specifically in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology. • 1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).
