📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with health plan members and build trusted relationships • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and intuitive clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Help bridge gaps in care and connect people to appropriate care • Educate members and answer their health questions • Visit people nationwide as part of a network of purpose-driven clinicians

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to perform in-home health evaluations using an iPad and clinical workflow • Ability to work 40 hours per week
