📋 Description

• Provide in-home health evaluations in members’ private residences • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Provide Signify Health services, including Diagnostic & Preventive Services • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Build trusted relationships and help improve health outcomes • Educate members and answer health questions without prescribing medication, ordering lab tests, or altering current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Ability to work safely and comfortably in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants • Flexibility to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Nurse Practitioner or Physician Assistant qualification
