📋 Description

• Provide in-home health evaluations for health plan members • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the person’s health plan, such as diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Visit people nationwide and connect them to appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Able to cover Rockingham/Strafford County • Able to travel locally and within licensed states as needed • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, airborne allergens, or other irritants • Anticipated schedule of 40 hours per week
