📋 Description

• Provide part-time in-home health evaluations to health plan members • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams, spirometry, and bone density screening • Build trusted relationships and help improve health outcomes • Identify care gaps and connect members to appropriate care • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services products • Travel locally and within licensed states as needed • Work primarily in members’ private residences

🎯 Requirements

• MD or DO qualification • Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants • Multi-state licenses preferred • Ability to use an iPad and clinical workflow for evaluations
