📋 Description

• Provide in-home health evaluations for health plan members • Conduct approximately 45-minute visits • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan • Use an iPad and clinical workflow for each evaluation • Visit people nationwide to connect them with appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services

🎯 Requirements

• MD or DO • Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow • Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
