📋 Description

• Provide in-home health evaluations in members’ private residences • Engage one-on-one with members and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic and preventive services when ordered, including diabetic eye exams, spirometry, and bone density screening • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Build trusted relationships and help improve health outcomes • Connect members to appropriate care and identify gaps in care

🎯 Requirements

• MD or DO qualification • Active, unrestricted license(s) in coverage area(s) • Ability to travel locally and within licensed states as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in members’ homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Multi-state licenses preferred • Ability to perform in-home health evaluations, including medication and medical history review and physical evaluation • Ability to use an iPad and clinical workflow
