📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with people and build trusted relationships to improve health outcomes • Conduct approximately 45-minute visits, including medication and medical history review and physical evaluation • Perform additional diagnostic and preventive services when ordered, such as diabetic eye exams, spirometry and bone density screening • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states, potentially across all counties in a state and other areas of business need • Conduct virtual visits as needed • Visit people nationwide and help connect them to appropriate care • Provide education and answer members’ health questions • Work as a clinician colleague with Signify Health

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Able to travel locally and within licensed states • Willingness and ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic tests as indicated
