📋 Description

• Provide approximately 45-minute In-Home Health Evaluations in members’ homes • Engage with members and build trusted relationships • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services, including spirometry, DEE, and bone density screening • Travel locally and within licensed states as needed • Help identify connections to appropriate care and bridge gaps in care • No requirement to prescribe medicine, order lab tests, or alter current treatment regimens

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Multi-state licenses (preferred) • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants • Ability to use an iPad and clinical workflow • Ability to safely perform in-home clinician duties
