• Provide diagnostic patient care primarily through virtual, remote consultation by video conference or telephone • Serve as the dispositional authority for after-hours and weekend clinical calls • Assess, diagnose, treat, and document patients’ physical and psychosocial health status • Assess acute and non-acute clinical problems through virtual video conference or telephone • Perform and document physical evaluations and patient histories • Analyze patient-condition trends and develop, document, and implement patient management plans • Develop plans of care including health education, physician referrals, case management referrals, and patient/family counseling • Plan patient care based on patient population knowledge and protocols • Collect and analyze comprehensive and focused health data for patients and families • Confer with PCPs and other medical providers to report health data and ensure guideline compliance • Ensure optimal patient outcomes through telemedicine and collaborate with on-call PCPs as needed • Evaluate progress toward expected outcomes and implement appropriate protocols • Enhance telehealth practices and systems using data demonstrating program effectiveness • Communicate through varied formats, tools, and technologies to deliver care across the continuum • Provide safe, effective, financially responsible services and extraordinary customer service • Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings • Adhere to departmental objectives, performance standards, regulatory requirements, quality-care compliance, policies, and procedures • Practice according to a written or electronic practice agreement • Participate in developing telehealth/telemedicine policies, procedures, and protocols • Initiate or participate in quality improvement activities • Participate in committees supporting growth • Provide feedback regarding others’ practice to improve patient care • Coordinate services with other programs • Perform other duties assigned by management
• Bachelor’s degree in Nursing (BSN) and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty required • Master’s degree in Nursing required • Board certification by AANP or ANCC required • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required upon hire • Current DEA number for Schedule II-V controlled substances may be required based on State of practice • Multi-state licensure including Florida and at least three licenses among VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA, and KS • Minimum 3 years’ acute/primary care clinical work experience required • Telehealth work experience preferred • Spoken and written fluency in English • Ability and willingness to obtain independent/autonomous practice as an NP in applicable states • Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook • Ability and willingness to travel up to 10% for meetings and trainings • Ability and willingness to travel to Georgia or other states requiring onsite observation and medical record review • Availability to work evenings/overnights and/or weekends depending on assigned schedule • Minimum availability to work four holidays per calendar year • Ability to conduct telemedicine visits in accordance with state and federal regulations • Expert knowledge of clinical functions, practices, processes, procedures, techniques, and methods • Knowledge of medical practices to function independently as a certified practitioner and collaborate with physicians and other medical providers • Demonstrated achievement of clinical performance metrics • Excellent clinical judgment and problem-solving ability • Ability to prioritize and work under pressure • Ability to communicate and collaborate professionally with physicians, patients, and team members • Ability to operate effectively with a multidisciplinary team
