📋 Description

• Provide In-Home Health Evaluations in patients’ homes • Engage with people one-on-one and answer health questions • Review medications and medical history • Conduct physical evaluations • Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Use an iPad and clinical workflow for each evaluation • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, DEE, and bone density screening • Help bridge gaps in care and connect people to appropriate care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Able to cover the primary location of Penobscot, ME • Nurse Practitioner or Physician Assistant qualification • Able to travel locally and within licensed states as needed
