📋 Description

• Provide in-home health evaluations in members’ homes • Travel primarily across a defined coverage area, up to 100% of the time • Build trusted relationships with health plan members and help improve health outcomes • Conduct approximately 45-minute visits • Answer health questions and review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Travel across all New Hampshire counties and flex to other areas based on business needs • Support connections to appropriate care

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within New Hampshire and flex to other areas of business need • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, airborne allergens, and other irritants • Able to travel locally and within licensed states, up to 100% of the time • Willingness and ability to stay overnight in hotels and use car rentals and/or flights as needed • Ability to conduct virtual visits as needed • Ability to provide diagnostic and preventive services, including spirometry and diabetic eye exams • Ability to use an iPad and clinical workflow
