📋 Description

• Provide In-Home Health Evaluations in members’ homes • Engage with members and build trusted relationships to improve health outcomes • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams, spirometry and bone density screenings • Use an iPad and clinical workflow for each evaluation • Answer health questions and provide health education • Travel locally and across licensed states, potentially up to 100% of the time • Conduct virtual visits as needed • Provide Signify Health services, including Diagnostic & Preventive Services products • Help connect members to appropriate care and bridge gaps in care

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Comfortable and safe working in homes that may contain household pets, secondhand smoke, airborne allergens or irritants • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states • Ability to travel up to 100% of the time, including possible overnight hotel stays, car rentals and/or flights • Ability to conduct virtual visits as needed • Ability to use an iPad and clinical workflow • Anticipated weekly hours: 40
