📋 Description

• Provide in-home health evaluations to health plan members • Engage with members in their homes and build trusted relationships • Conduct approximately 45-minute visits including medication and medical history review and physical evaluation • Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams and spirometry • Conduct virtual visits as needed • Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening • Travel locally and within licensed states as needed • Help identify care gaps and connect members to appropriate care • Join a national network of clinicians focused on improving health outcomes

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Ability to travel locally and within licensed states as needed • Ability to conduct visits in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations
