📋 Description

• Deliver telephonic medication therapy management (MTM) to high-risk members • Perform telephonic comprehensive medication reviews and complete medication reconciliations • Conduct clinical assessments for applicable conditions • Engage and HIPAA-authenticate members for comprehensive medication reviews • Update member mailing addresses and document medication allergies and side effects • Verify prescriptions, vitamins, supplements, and over-the-counter medications • Identify, document, and facilitate resolution of medication-related problems • Create written patient takeaways at an appropriate health-literacy level • Create faxed materials for prescriber offices to facilitate resolution of medication-related problems • Build and maintain positive relationships with physicians, nurses, internal and external customers • Interact with patients, prescribers, nurse case managers, clients, and internal resources to develop member-specific treatment plans • Apply nationally recognized treatment guidelines • Complete internal training in behavioral modification and interviewing • Perform other responsibilities as directed by the supervisor and/or manager

🎯 Requirements

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform • 7 Months within the MTM Ops department and Meets Expectations in all core metrics • Previous experience in a call center environment • Experience maintaining focus and meeting goals in a work from home environment • Excellent spoken and written communication skills • Basic computer skills and proficiency with Microsoft Office products • Exceptional prioritization and organizational skills • Acts with integrity and uses sound judgment in dealing with confidential information • Applicant must reside and have an active RPh license in the state of PA, IL or FL • BS in Pharmacy or PharmD • Active pharmacist license in good standing
