• Responsible for overall care management and quality of care for participants. • Review assessments and discipline-specific documentation for quality, address deficiencies, and monitor field staff documentation. • Represent the physical therapy discipline in care planning meetings. • Coordinate holistic care plans and a continuum of care with discipline-specific staff and the interdisciplinary team (IDT). • Arrange, coordinate, and authorize appropriate member-specific services, including ADL/IADL assistance, housing, meals, transportation, and community-based services. • Conduct coaching, competency, and training sessions with field staff. • Coordinate transitions between care settings with team members, facilities, discharge planners, and home care coordinators. • Document services according to CLHC standards and federal/state regulations. • Arrange long-term care services in nursing homes, rehabilitation facilities, and other settings. • Collaborate with physicians, specialty services, and IDT members regarding changes in participant condition. • Provide or arrange skilled services, service authorization, periodic assessments, reassessments, and evaluations. • Monitor member responses, service utilization, care-plan effectiveness, and outcomes; modify plans as needed. • Maintain current knowledge of health issues, procedures, evidence-based guidelines, medications, and practice standards. • Recommend improvements to services, programs, policies, and procedures. • Follow disciplinary policies and engage Human Resources as needed. • Act within scope of practice, meet PACE competencies before independent work, and serve as an IDT member.
• Graduated from a Physical Therapy program acceptable to New York State Education Department (NYSED.) • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), Rehab, long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred. • Customer Service experience required. • Managed long-term care insurance experience beneficial. • Minimum of one (1) year of experience working with a frail or elderly population, or receive appropriate PACE organization training upon hiring if lacking this experience. • Supervisory experience preferred. • Be legally authorized to practice in the State in which the healthcare professional will perform the function. • Be medically cleared for communicable diseases and have all immunizations up-to-date before direct participant contact. • Current active and unrestricted license and registration in New York State required. • Bilingual preferred. • Ability to meet stated physical, mobility, lifting, visual, hearing, communication, and cognitive requirements.
