📋 Description

• Deliver telephonic medication therapy management (MTM) for high-risk members • Perform telephonic comprehensive medication reviews and medication reconciliations • Conduct clinical assessments for applicable conditions • Engage and HIPAA-authenticate members for comprehensive medication reviews • Update member addresses and document medication allergies and side effects • Verify prescriptions, vitamins, supplements, and over-the-counter medications • Identify, document, and facilitate resolution of medication-related problems • Create written patient takeaway summaries at an appropriate health-literacy level • Create faxed materials for prescriber offices • Build and maintain relationships with physicians, nurses, members, caregivers, prescribers, clients, and internal resources • Develop member-specific treatment plans based on nationally recognized treatment guidelines • Complete internal training and apply behavioral modification and interviewing skills • Perform other responsibilities as directed by the supervisor or manager

🎯 Requirements

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform • 7 Months within the MTM Ops department and Meets Expectations in all core metrics • Previous experience in a call center environment • Experience maintaining focus and meeting goals in a work from home environment • Excellent spoken and written communication skills • Basic computer skills and proficiency with Microsoft Office products • Exceptional prioritization and organizational skills • Acts with integrity and uses sound judgment in dealing with confidential information • Applicant must reside and have an active RPh license in the state of PA, IL or FL • BS in Pharmacy or PharmD • Active pharmacist license in good standing
