📋 Description

• Use a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and influence members regarding chronic-condition health goals • Assess members using clinical tools and information/data review • Evaluate members' needs and benefit-plan eligibility for integrated internal and external programs and services • Determine members' health literacy, technology capabilities, and readiness to change • Apply and interpret criteria, guidelines, health/wellness management plans, policies, procedures, and regulatory standards • Consult with supervisors and others to overcome barriers to goals and objectives • Identify and escalate quality-of-care issues through established channels • Use influencing and motivational-interviewing skills to maximize member engagement and promote lifestyle and behavior changes • Provide coaching, information, and support to empower independent medical and healthy-lifestyle choices • Interpret and use clinical guidelines and provide current healthcare information • Help members participate knowledgeably with providers in healthcare decision-making • Develop and monitor plans of care in collaboration with members and/or attending physicians • Use internal policies and procedures in compliance with regulatory and accreditation guidelines

🎯 Requirements

• Must be a CDCES (Certified Diabetic Educator) • Active and unrestricted RN licensure in the state of residence • State of residence must be a compact state • 3+ years of clinical experience • Must possess or be willing and able to obtain high speed broadband internet access • Previous coaching experience (preferred) • Managed care experience (preferred) • Experience with Microsoft Word, Outlook, Excel, and comfort using various computer programs (preferred) • Willing to obtain additional nursing licensure (preferred) • Associate's degree required • BSN preferred • Monday-Friday standard business hours, including 1 evening shift per week from 12:30 pm EST
