📋 Description

• Partner with a registered nurse case manager in a collaborative process of assessment, planning, and advocacy • Meet members’ benefit plan and/or health needs • Research available options and resources for members with financial, transportation, safety, and housing needs • Provide telephonic and electronic member contact across the entire lifecycle population • Empower members to make positive lifestyle and medical choices • Collaborate with clinical staff to assist members with medical, financial, and behavioral health community resources • Work Monday-Friday 8am-4:30pm EST • Provide no weekend, holiday, or travel coverage

🎯 Requirements

• Must live in North Carolina • Master's Degree in Social Work • Active, unrestricted licensure as one of the following: LSW (Licensed Social Worker), LMSW (Licensed Master Social Worker), LCSW (Licensed Clinical Social Worker), LISW (Licensed Independent Social Worker) • 2+ years of experience in medical social work or case management • Ability to obtain and maintain multi-state licensure (minimum of 10 states – willing to obtain additional licenses) before completing new hire training • Must be able to work EST hours • Monday-Friday 8am-4:30pm EST availability
