📋 Description

• Provide effective and equitable value-based primary care to adults on Medicare via telemedicine • Complete same-day sick visits, triage acute escalations, post-discharge visits, chronic disease management, and annual wellness assessments • See patients independently and collaborate with physicians based on patient needs • Utilize evidence-based practice to provide virtual care • Conduct annual wellness visits and health risk assessments with accurate, specific documentation • Conduct visits via video and phone • Triage acute escalations from the call center and support next steps in management • Coordinate care with primary care providers, licensed medical social workers, behavioral health specialists, specialty care, and testing facilities • Track patient outcomes using a population health focus and participate in regular outcome reviews • Educate caregivers and patients on preventative care, lifestyle modifications, and self-management strategies • Adjust to the evolving telehealth program and support Oak Street’s care model • Obtain and maintain multiple state licenses according to business needs • Perform other duties as assigned • Report to the Central Telehealth Medical Director and work closely with operational leadership

🎯 Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate • National certification in at least one of: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner • Active, non-probationary state Nurse Practitioner license • Ability to obtain new licenses as needed • US Work Authorization • Willingness to obtain and maintain multiple state licenses, including RN, NP, CSR, and DEA, according to business needs • 3–5 years of experience in primary care, including internal medicine, geriatrics, or family medicine • 1–2 years of virtual care experience preferred • Licensure in AL, AR, AZ, CO, IA, IL, IN, LA, NM, OK, TN, or TX strongly preferred • Willingness to work across MT, ET, and CT time zones • Spanish language skills strongly preferred • Proficiency using telehealth platforms and technology • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments • Passion for teamwork and cross-functional collaboration • Interest in an innovative, empirically guided population-health model • Someone who embodies being “Oaky”
