📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical histories • Educate members about their health and answer questions • Conduct Video Visits from a private space with adequate internet connection • Maintain 40 hours of weekly availability and 320+ monthly capacity units • Maintain a strong visit completion rate of 220+ completed visits per month • Attend regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Provide telehealth services only • Interact remotely with health plan members using computer, phone, iPad, and other electronic devices

🎯 Requirements

• Full-time, board-certified nurse practitioner • Active, unrestricted license(s) in coverage area(s) • Board certification required • Able to obtain and maintain multi-state licenses in coverage area, as required • FNP certification; AGNP certification preferred for NPs • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Maintain training, affiliation, and compliance with Video Visit requirements • Provide 40 hours of availability weekly • Consistently provide 320+ capacity units per month • Consistently achieve 220+ completed visits per month • Private space with adequate internet connection • Broadband connectivity with video and audio enabled • Quiet setting with neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology
