📋 Description

• Lead chronic kidney disease (CKD) classes in-person, virtually, and telephonically • Educate patients and caregivers about kidney function, disease progression, diet and medication management, and renal replacement options • Teach treatment modalities including Peritoneal Dialysis, Home Hemodialysis, In-Center Hemodialysis, and Transplant • Build relationships with local nephrologists, clinical practices, and dialysis care teams • Serve as central point of contact for the Westside Division Kidney Smart team • Assist the Division Lead and field operations in expanding educational reach • Coordinate and publish class calendars • Manage room and venue logistics and educator coverage • Manage patient recommendation and physician referral intake pipelines • Conduct patient outreach and enrollment • Conduct post-class follow-up and track attendance metrics • Submit required reporting tools and ensure alignment with national Kidney Smart operational standards

🎯 Requirements

• Active clinical credential/license required (e.g., Certified Clinical Hemodialysis Technician / CCHT, LPN, LVN, RN, or equivalent clinical licensure) • Strong background in nephrology, dialysis, or chronic disease patient education preferred • Bilingual in English and Spanish strongly preferred • Exceptional public speaking, teaching, and interpersonal skills • Ability to connect with diverse patient populations • Strength in scheduling, workflow coordination, administrative tracking, and cross-functional team support • Reliable transportation • Willingness to travel up to 20% within the assigned Phoenix metropolitan territory
