📋 Description

• Serve as the point of contact for incoming patient referrals to the Kidney Palliative Care Program • Conduct medical record reviews and collaborate with the care team to determine advance care planning needs for CKD and ESKD patients • Provide actionable guidance and next steps to the palliative care RN or Social Worker for warm, efficient handoffs • Communicate across the interdisciplinary team regarding patient enrollment and plans of care • Provide timely education and guidance to clinical peers • Identify trends and educational opportunities • Partner with leadership on quality improvement initiatives • Triage complex kidney patients and connect them with appropriate palliative resources • Help grow DaVita IKC’s national kidney care palliative program

🎯 Requirements

• Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) with an active, unencumbered license • Graduate from an accredited school of nursing or social work required • Minimum of ten (10) years’ experience in clinical nursing or social work required • Hospice or palliative care experience required • Renal nursing or social work experience preferred • Expertise in medical record interpretation, patient assessment, and care planning • Ability to provide clear, consultative feedback to clinical peers • Ability to identify trends, solve problems, and improve processes collaboratively • Ability to work independently and manage time effectively in a dynamic remote environment • Basic computer skills and proficiency in MS Word and Outlook required • Home office with internet connectivity of at least 1MB upload and 1MB download speed required • Ability to work Monday-Friday • Must work from a home office • Must be eligible to work anywhere in the continental United States
