• Serve as a consistent clinical point of contact for each member throughout their wellness journey • Assess members’ immediate risk and clinical needs using evidence-based models • Provide real-time crisis de-escalation, safety planning, and risk management • Identify biopsychosocial needs and determine solutions and next steps with members • Develop SMART goals supporting healthy lifestyles • Assess readiness to change and implement actions through stages of change • Shepherd members to appropriate clinical care, treatment programs, and community resources • Educate members about benefit plans, coverage, and care navigation • Apply specialized evidence-based curriculum across clinical areas • Research and provide tailored educational or support resources • Partner with internal colleagues for follow-up, case collaboration, and care transitions • Balance real-time queue work with case management responsibilities • Demonstrate diplomacy, empathy, and professionalism in difficult conversations • Comply with HIPAA, confidentiality, compliance, and clinical documentation standards • Manage multiple health records and systems to document interactions and progress • Communicate effectively in verbal and written formats • Perform other related duties
• Master’s degree or PhD in a mental health field, or RN with psychiatric specialization • Current independent clinical license, such as LCSW, LPC, LMFT, LPCC, PhD, PsyD, PMHNP, or psychiatric RN license • Minimum 5 years of post-licensure behavioral health or substance use clinician experience preferred • Demonstrated use of brief therapy, cognitive-behavioral therapy, and motivational interviewing • Managed care experience preferred • Direct member communication experience; telephonic counseling/coaching experience preferred • Required to obtain AAS Crisis Specialist Certification within the first year of employment • EAP experience and CEAP certification preferred but not required • Strong clinical acumen and interpersonal communication skills • Comfort working in a dynamic, rapid-paced environment • Proven administrative abilities and strong computer/software application skills • Minimum 40 hours per week; rotating shifts, weekends, and holidays • Home internet through cable broadband or fiber optic service with at least 10 Mbps download and 5 Mbps upload
