• Lead the team of IKC RNs who case manage dialysis and CKD patients • Lead operational execution of new care models for CKD Medicare Advantage patients • Oversee quality and performance measures, including STARs metrics • Support RN hiring and oversee onboarding • Provide clinical guidance and training to RNs • Serve as a key leader of clinical and operational strategy with senior and cross-functional leadership • Manage Disease Management programs in the designated market or geography • Review and analyze data to drive patient care outcomes • Build and manage relationships with internal and external clients • Provide clinical support to Registered Nurse Case Managers and direct nursing care as needed • Lead direct reports and manage employment/HR activities, including payroll, PTO, hiring, onboarding, professional development, performance management, and disciplinary actions • Ensure compliance with regulations and DaVita policies • Monitor risk management and quality assurance compliance • Achieve quality and metric goals in Integrated Kidney Care programs • Coordinate activities with DaVita IKC management, Regional Medical Directors, cross-functional departments, and nurses • Evaluate processes for cost effectiveness and reduce operational expenses • Implement initiatives and analyze their impact on outcomes • Create and submit reports and analyses of teammate performance • Travel within the market and occasionally outside the market
• Minimum of 5 years management experience leading and managing teams required • Experience managing to data-driven outcomes and process improvement required • Primary Care and value-based care experience required • Strong communication on medical management, operational, and business-related issues • Minimum of 5 years experience in clinical nursing leadership preferred • Registered Nurse (RN) licensure required • Intermediate computer skills • Proficiency in MS Word, Excel, PowerPoint, and Outlook required • Home office with Internet connectivity of at least 1MB upload and 1MB download speed required • Must live in Mountain or Central time • Ability to travel as needed • Unencumbered Registered Nurse licensure needed
