📋 Description

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients • Minimize fragmentation of the healthcare delivery system • Function autonomously and collaborate with the healthcare team • Coordinate care among patients, families, physicians, providers, care teams, community resources, and the Regional Operations Manager • Support ESKD and CKD patients throughout California, Oregon, Nevada, and Washington as needed • Accommodate patient needs and nephrology partner availability Monday through Friday

🎯 Requirements

• ADN degree from an accredited school of nursing required; a three-year diploma from an accredited diploma program may be substituted for nursing degree • BSN preferred • Current Registered Nurse (RN) license in the state of practice required; must hold or obtain licensure in California, Oregon, Nevada, or Washington as applicable • Continuing education credits maintained as required by state of practice • Minimum of five years’ experience in clinical nursing strongly preferred • Minimum of three years’ experience in renal nursing preferred • Case management experience required, or 3+ years assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred • Knowledge of data and management of clinical, financial, and patient satisfaction outcomes • Experience and effectiveness in a change agent role • Knowledge of CQI techniques • Previous healthcare performance coaching experience required • Current CPR certification required • Ability to adapt to new/electronic processes and increased productivity expectations for Capella implementation • Current driver’s license in state employed, positive driving record, and ability to meet insurance coverage requirements • Basic computer skills and proficiency in MS Word and Outlook • Functional proficiency with DaVita clinical software programs, including Capella, within 90 days of employment • Home office with internet connectivity of at least 1MB upload and 1MB download speed
