📋 Description

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients • Minimize fragmentation of the healthcare delivery system • Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager • Work autonomously and collaboratively with healthcare team members • Assess patient needs and develop and manage individualized care plans • Coordinate services across interdisciplinary teams • Support patients through complex healthcare transitions • Meet business and patient needs through travel within the assigned territory • Attend nephrology partner meetings monthly or quarterly as needed • Adapt territory assignments based on business needs

🎯 Requirements

• A.D.N degree from an accredited school of nursing required; B.S.N preferred; three-year diploma from an accredited diploma program may be substituted for nursing degree • Current Registered Nurse (RN) license in the state of practice required • Continuing education credits maintained as required by state of practice required • Minimum of five (5) years’ experience in clinical nursing required • Minimum of three (3) years’ experience in renal nursing preferred • Case management experience strongly preferred, including assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred • Knowledge of data and management of clinical, financial, and patient satisfaction outcomes • Experience and effectiveness in a change agent role • Knowledge of CQI techniques • Previous healthcare performance coaching experience required • Current CPR certification required • Ability to adapt to new/electronic processes and increased productivity expectations for Capella implementation • Current driver’s license in the state employed, positive driving record, and ability to meet insurance coverage requirements required • Basic computer skills and proficiency in MS Word and Outlook required • Functional proficiency with DaVita clinical software programs, including Capella, required within 90 days of employment • Home office with internet connectivity of at least 1MB upload and 1MB download speed required
