📋 Description

• Provide triage, case management, and care navigation for oncology patients with potential comorbidities • Deliver direct patient care through phone, email, video, and virtual platforms • Provide comprehensive support throughout the oncology care journey, from screening through survivorship or end-of-life care • Provide urgent clinical escalations and consultative clinical support • Triage symptom survey responses and inbound reported symptoms, supporting patients directly or escalating to Nurse Practitioners • Identify and address patient needs including care coordination, nutritional support, discharge planning, and provider referrals • Educate patients about their diagnosis, Daymark Health's care offerings, and next steps • Build trusting relationships with patients and their care networks through empathy and active listening • Coordinate with Nurse Practitioners, Medical Directors, Social Workers, Clinical Operations, and non-clinical Health Partners • Address social determinants of health such as food security and transportation access • Follow Daymark policies, procedures, and documentation standards and optimize use of systems, tools, and technology

🎯 Requirements

• Bachelor of Science Degree in Nursing required • Compact unrestricted registered nurse (RN) license, or ability to obtain • Licensure in Rhode Island, or willingness to become licensed in Rhode Island • At least 3 years of nursing experience, including 2 years in oncology nursing • Passion for helping patients understand the benefits of palliative care • Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM strongly preferred, or to be obtained within 2 years of hire • Comfort using Slack, Zoom, Google Suite, and EHR systems • Strong relational, communication, organizational, and adaptability skills • Ability to multitask in fast-paced environments while maintaining meticulous organization in communications and documentation • Basic Life Support (BLS) certification required for patient-facing roles by the start date • Compliance with Daymark's Employee Vaccination and Testing policy required by the start date
