📋 Description

• Screen members for Diabetes Clinic eligibility and appropriateness • Complete detailed assessments and follow up with members to provide ongoing education on disease process, medications, glucose monitoring, lifestyle counseling, and contingency planning • Schedule follow-up appointments with self and providers • Connect members and providers with the right resources within Devoted to troubleshoot clinical issues • Create and achieve collaborative SMART goals with members • Use motivational interviewing to influence positive sustained behavioral changes • Identify gaps in care and provide resources • Collaborate with nurse practitioners, social workers, and pharmacists to develop plans of care and optimize medication regimens • Maintain documentation related to member and provider interactions • Continue improving skills through education series and continuing education providers

🎯 Requirements

• Active unrestricted Compact Registered Nurse license or unrestricted Registered Dietitian license • Ability to obtain additional licenses in states where Devoted operates as needed • Active Certified Diabetes Care and Education Specialist (CDCES) certification • At least 2 years of experience clinically managing diabetes • Remote experience managing diabetes strongly preferred • Experience working with diabetes technology, including CGMs and insulin pumps • Bilingual Spanish/English capability
