📋 Description

• Deliver exceptional, empathetic clinical care through real-time and asynchronous chat and other responsive care channels • Manage multiple concurrent and asynchronous patient conversations with accurate documentation and follow-through • Guide and oversee patients’ medical management using a holistic framework, including the 5M approach • Collaborate with primary care providers, specialists, pharmacy, and specialty care teams • Coordinate care with patients’ family members and caregivers when appropriate • Assess symptoms, medication questions, lab results, and new health concerns • Escalate patients to telephone, video, in-person, urgent, or emergency care when appropriate • Maintain ownership of patient needs through resolution or documented handoff • Maintain accurate and timely EHR documentation in compliance with legal, ethical, and telehealth regulations • Identify opportunities to improve patient experience and outcomes through digital workflows and proactive communication • Contribute to digital workflows, standardized responses, escalation pathways, and quality standards • Occasionally support broader Devoted Medical service lines • Provide flexible clinical coverage across Responsive Care Department queues and on-call services • Adhere to relevant laws, regulations, patient privacy requirements, and telehealth standards

🎯 Requirements

• Nurse Practitioner or Physician Assistant advanced practice provider • Three or more years of experience in primary care, house calls, internal medicine, family medicine, geriatrics, palliative care, and/or urgent care • Ability to review AI-generated responses for clinical accuracy and safety • Nurse Practitioners: active and in-good-standing RN and APRN licenses • Physician Assistants: active and in-good-standing Physician Assistant license • Role licensure and certification in good standing required • Ability to obtain licensure in requested states within 90 days of hire date, starting at offer letter signing • Additional state licensure as needed • Prior Devoted Medical experience preferred • Prior virtual care or telehealth experience preferred • Experience in real-time, queue-based, or high-volume telehealth clinical environments • Experience collaborating with interdisciplinary care teams and coordinating care remotely • Comfort managing complex or vulnerable patient populations in a digital-first setting • Ability to thrive in a fast-paced, rapidly evolving environment with flexibility and curiosity • Ability and willingness to work four 10-hour shifts per week • Working hours of 8am–6pm MST/PST, with consideration for EST/CST hours • Compliance with applicable laws, regulations, patient privacy, and telehealth standards
