📋 Description

• Deliver virtual acute care to members with emergent/critical illness • Diagnose complex medical conditions • Order and interpret diagnostic tests • Establish care plans with patients, families, and the Care OnDemand team • Perform Care OnDemand visits, including evaluating and diagnosing acute illnesses • Prescribe appropriate medications • Assess quality-of-care interventions, social needs, home health needs, and DME needs • Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers • Use the electronic health information system for visits and provide feedback to improve the interface • Support clinical nurses through weekend on-call coverage in certain geographies • Support new clinical and organizational programs and initiatives • Integrate innovative virtual care practices

🎯 Requirements

• At least three years or more working as an APP, preferably in acute settings, urgent care, emergency medicine, and/or primary care or internal medicine with heavy acute management • Must be willing to work four 10-hour shifts per week, including one weekend shift per month, from 11am-9pm EST • Bilingual in Spanish/English • For Nurse Practitioners: active and in-good-standing RN and APRN licenses • For Physician Assistants: an active and in-good-standing Physician Assistant license • Role licensure and certification in good standing is required • Ability to get licensed in requested states within 90 days of hire date • Willingness to obtain licenses in additional states as needed • Experience in primary care, internal medicine, urgent care, emergency room, and/or geriatrics • Experience performing visits over telehealth video platforms • Experience managing acute/chronic disease exacerbations including CHF exacerbations, diabetic emergencies, COPD exacerbations, and hypertensive emergencies • Strong desire to continue practicing acute care • Adaptability, eagerness to learn, and comfort working in a fast-paced, dynamic setting • Flexibility in scope of practice and willingness to embrace new challenges
