📋 Description

• Work directly with patients to manage chronic conditions through personalized care plans • Develop and provide educational materials tailored to the specific needs of the local population • Collaborate with interdisciplinary teams to iterate on disease management processes • Help members and providers navigate benefits to ensure healthcare is accessible and affordable • Conduct comprehensive medication reviews for members with complex medication regimens • Identify and resolve drug therapy issues to optimize care and improve outcomes • Drive “5-Star” performance on pharmacy-related clinical measures • Address therapy gaps, including statin initiation for diabetes or cardiovascular health • Interact compassionately with members to help them reach individual health goals • Support on-call rotations for urgent pharmacy needs • Act as a clinical subject matter expert for pharmacy model development • Mentor and train the wider team on pharmacotherapy and practice-related topics • Utilize AI-enabled clinical tools for documentation and drug therapy problem identification, providing feedback to improve accuracy and workflow efficiency • Secure additional state licensure in a timely manner • Perform other duties as assigned • Report to the Senior Manager, Clinical Pharmacy, with indirect reporting relationships to other clinical and operational leadership

🎯 Requirements

• 3 or more years of clinical experience working in direct patient care • State licensure in good standing is required in Alabama, Florida, Georgia, Hawai’i, Indiana, Missouri, Ohio, North Carolina, South Carolina, or Texas • Required to get licensed in at least 5 states and additional states as needed • License must be in good standing without restrictions or sanctions • Travel may be required to secure additional licensure • Experience with implementing or working under collaborative pharmacy practice agreements preferred • Board certification or experience specifically in complex geriatric care, behavioral health, and/or lipid management preferred • Comfort working with AI-assisted clinical tools and providing feedback to improve their use • Licensure in 3 or more specified states at the time of application desired
