📋 Description

• Conduct outreach via telephone and SMS to educate and inform members about clinical programs and relevant screenings • Ensure patients remain connected to care throughout the program • Coordinate care delivery with the multidisciplinary care team • Schedule initial appointments and adjust calendars to meet urgent care needs • Reschedule visits for other team roles • Provide inbound call support and route calls for patients with clinical or administrative questions • Communicate frequently with care team members and leadership • Participate in daily care team coordination huddles and patient rounds • Develop understanding of benefit offerings to investigate and resolve member questions • Document accurate information about member needs in the system • Support daily administrative tasks • Pilot new processes and workflows • Work with other Devoted teams to address member needs

🎯 Requirements

• Minimum of 3 years of healthcare experience in a customer/patient-serving role, such as a Care Coordinator • Must be willing to work Monday-Friday, 8:30am-5pm Central Standard Time (CST) • Bilingual Spanish speakers and experience engaging across various cultures • Health insurance experience (Medicare Advantage a plus) • Healthcare experience in scheduling, benefits, care coordination, medical terminology, and HIPPA regulations a plus • Availability to work variable shifts to support the needs of members across multiple time zones • Proficient computer skills and ability to learn new systems • Google Suite proficiency, including Google Drive, Google Docs, Google Sheets, and Google Slides, a big plus • Great communication, problem solving, analytical, and documentation skills • Effective time management and organization skills • Ability to empathize with patients, relatives, and colleagues with dignity and respect
