📋 Description

• Conduct daily phone calls with prior patients in the assigned territory to follow up on health status and potential changes in condition warranting additional services • Connect patients with appropriate internal team members based on patient needs and requests, including skilled care, personal care, hospice care, and ElaraConnect personnel • Answer inquiries regarding enterprise services and provide appropriate information • Collaborate with Patient Care Specialists for leads identified in the assigned market territory • Collaborate with Patient Care Specialists and service line operators to resolve Check-In operation issues • Remain current on virtual customer relations trends and innovations • Assist in evaluating agency operations and systems and identify ways to enhance workflow and productivity • Participate in clinical record audits, utilization reviews, Peer Review, and Quality Improvement programs • Attend training, education, seminars, or other learning opportunities • Notify the supervisor of problems requiring administrative attention and intervention • Process data entry information accurately into company systems • Report to the Program Supervisor

🎯 Requirements

• High School Diploma/GED • 1+ year experience in a home health care setting preferred • Knowledge of Medical Terminology • Proficiency with MS Office Suite (Word, Excel, Outlook) • Excellent verbal and written communication skills • Participation in weekends and/or holidays, as assigned
