• Oversee daily inbound calls, fax referrals, emails, and department tasks • Assist Care Managers with assigned tasks • Monitor patient care through assessment and evaluation to ensure progress toward desired outcomes • Identify and resolve barriers to effective patient care • Improve the quality and completeness of documentation • Collaborate with patients, families, nurses, physicians, and other team members • Document clearly, completely, concisely, and in an organized manner • Monitor clinically low-risk cases • Advise the Care Manager and/or Medical Director of new or concerning clinical issues • Institute necessary actions to promote quality and appropriate care • Accept referrals and identify patients meeting care-management criteria • Support HOPCo’s values and mission • Assist with program planning, development, and evaluation • Perform related and other CIN duties as assigned
• Medical Assistant or Certified Nursing Assistant degree or equivalent working knowledge preferred • Minimum three (3) years of certified nursing assistant or medical assistant experience preferred • One-year experience in a medical business office, physician’s office, or hospital environment preferred • Experience in value-based care / population health preferred • Experience in orthopedics or musculoskeletal specialty preferred • Knowledge of the health care field and orthopedics • Knowledge of policies and procedures concerning patient care • Knowledge of patient confidentiality, state laws, HIPAA regulations, and medical records procedures • Knowledge of medical practices, terminology, and reimbursement policies • Knowledge of Microsoft Office applications • Skill in follow-up with patients and responding to inquiries promptly • Skill and professionalism when interacting with providers, patients, family members, and the public • Skill in understanding patient education needs by effectively sharing information with patients and families • Skill in problem-solving and time management • Skill in evaluating the effectiveness of existing methods and procedures • Skill in verbal and written communication • Ability to read, interpret, and apply policies and procedures • Ability to set priorities among multiple requests • Ability to interact effectively with patients, medical and administrative staff, and case managers • Ability to work in a fast-paced environment • Must read and sign HOPCo Mission, Vision, and Values
