📋 Description

• Serve as a liaison/connector between patients, care teams, and the community • Help bridge conversations with patients and remove barriers to health and social services • Conduct telephonic outreach to patients for appointment scheduling, needs assessment, and care gap closure • Identify social determinants of health affecting patients’ health and well-being • Collaborate with social workers and/or care managers to develop action plans • Build trusting relationships through patient support and advocacy • Assist patients with health-related services and community resources • Accompany patients to specialist appointments and assist with enrollment forms • Facilitate communication among patients, families, colleagues, and community-based organizations • Document patient interactions in medical records • Attend required meetings • Travel to worksites and other locations as necessary

🎯 Requirements

• H.S. diploma or equivalent • Minimum of 1 year of experience in a healthcare, community-based, or social work environment • Experience utilizing electronic medical records • Any combination of education and experience providing an equivalent background • Ability to work an 8-hour shift between 8am and 6pm Pacific or Mountain Time • Ability to work within commuting distance from an office unless accommodation is granted • Bilingual preferred • Certified Community Health Worker preferred • Ability to travel to the worksite and other locations as necessary • COVID-19 and Influenza vaccination may be required for certain patient/member-facing roles
