📋 Description

• Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care • Provide clinical and non-clinical support to patients • Provide urgent healthcare via telephone and tele-video to patients receiving home and community-based services • Develop and implement clinical plans of care for adults with chronic and complex conditions • Gather patient histories, perform physical examinations, and obtain diagnostics as needed • Develop and implement treatment plans based on patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families on medications, side effects, illness progression, diet and nutrition, adherence, and crisis prevention • Coordinate care and resources with clinical team members and medical providers • Maintain patient medical records and documentation according to regulations and company policy • Participate in required continuing education • Prescribe medication as permitted by state authority

🎯 Requirements

• Requires an MS in Nursing • Requires an active national NP certification • Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in Texas, Iowa, or Kansas • Requires valid, current, active, RN Compact license • Requires 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) required • Possession of DEA registration or eligibility preferred • Active Medicaid number preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Monday-Friday, 8:00am-5:00pm CST and rotating on-call availability • Candidates must be within a reasonable commuting distance from the posting location(s), unless an accommodation is granted as required by law • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
