📋 Description

• Conduct health evaluations assessing substance use, co-occurring conditions, and overall functioning • Diagnose health conditions and create individualized treatment plans • Prescribe and manage medications, monitor side effects, and adjust treatment as needed • Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care • Serve as a clinical consultant on complex or high-need cases • Educate clients and families on treatment plans, medications, and recovery support strategies • Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards • Participate in team meetings, professional development, and continuous training • Provide telehealth-based assessments, medication management, and care coordination for individuals with SUD

🎯 Requirements

• Active and unrestricted NP license in Missouri • Active DEA license (and CDS where applicable) • Several years of experience in behavioral health, addiction medicine, or psychiatry • Experience in addiction medicine or psychiatry • Board certification in specialty preferred • Strong documentation skills and proficiency in EHR platforms • Compassion toward individuals living with SUD and mental health challenges • Ability to collaborate in integrated and team-based care models • Organizational and flexibility skills for navigating complex systems • Clear and confident virtual communication with clients and team members • Multiple and additional state licenses are a plus
