📋 Description

• Educate patients by phone about palliative care • Schedule patients for palliative appointments • Interface directly with patients, providers, and families • Work with clinical files to close out palliative visits and enter notes into systems • Spend part of the day on phones and part on administrative work • Help patients understand the value of palliative care • Connect patients in need of the program with palliative care providers • Answer patient questions about palliative care and the program • Confirm patient encounters are documented and patient needs are triaged • Follow up with patients to ensure they receive high-quality care from Empassion’s provider network • Review and close completed cases daily • Balance time between clinical triage and patient calls

🎯 Requirements

• Minimum 3+ years as an LPN • Minimum 3+ years of reading and updating clinical notes • Minimum 1+ years of relevant work experience in a customer service call center • Able to receive calls from patients, caregivers, and providers • Strong communication and written skills • Detail oriented • Comfortable working with technology and in a dynamic startup environment • Secure Wi-Fi connectivity • Spanish-speaking a plus but not required
