📋 Description

• Support chronic care management patient enrollment and onboarding • Verify patient eligibility and complete consent documentation • Maintain patient engagement and escalate inactive or non-engaged patients • Conduct weekly text and telephone outreach to enrolled CCM patients • Meet engagement-time standards aligned with Medicare CCM billing requirements • Reinforce provider-directed care plans and identify adherence barriers • Perform high-volume outbound calling, texting, and follow-up outreach • Document patient interactions in CCM and EHR systems • Track engagement time and maintain audit-ready documentation • Ensure HIPAA and PHI compliance • Facilitate access to referring providers and clinical staff • Support care transitions after hospitalizations or ED visits • Coordinate referrals and follow-up appointments • Escalate symptom concerns or medication issues to licensed clinical staff • Manage patient messaging workflows and collaborate with remote clinical and operational teams

🎯 Requirements

• Certified Medical Assistant • 1–3 years healthcare experience • Strong clinical assessment, communication, and documentation skills • Comfortable working in an EHR and CCM platform simultaneously • Ability to manage a structured, metric-driven workflow with reliability • Intermediate MS Office Suite skills, including Outlook and Teams • Ability to work primarily remotely • Home internet supporting at least 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect PHI/HIPAA information • Chronic disease or nephrology experience preferred • Experience in chronic disease management preferred • Prior work in virtual care, remote nursing, or telehealth programs preferred • Bilingual Spanish/English preferred • Familiarity with CCM regulatory requirements preferred • Strong patient education and motivational interviewing skills preferred • Must be able to work 8:00am–5:00pm Central Time
