📋 Description

• Conduct outreach to patients/families to promote access to primary care, preventive services, and community resources • Assist patients/families with healthcare-system navigation, appointment scheduling, referrals, and support services • Build trusting relationships to support engagement in care and adherence to treatment plans • Identify barriers to care, including transportation, insurance, food insecurity, and housing, and connect patients/families with appropriate resources • Provide culturally appropriate health education on chronic disease management, preventive health, medication adherence, and wellness • Support care management staff and clinical teams through follow-up calls and care coordination • Document patient interactions, services, and outcomes in EHR or designated systems • Participate in multidisciplinary team meetings and case conferences • Maintain confidentiality and professional boundaries while fostering patient empowerment and self-advocacy • Participate in continuing education, training, and quality improvement initiatives • Process and track incoming and outgoing referrals • Maintain scheduled work hours and reliable attendance • Perform other duties as assigned

🎯 Requirements

• High school diploma or equivalent required • Associate’s degree in health, human services, or related field preferred • Minimum of two years’ experience in a healthcare or social service setting, preferably in care coordination, case management, or patient navigation • Knowledge of healthcare systems, community resources, and social determinants of health • Proficiency with computers and electronic health record (EHR) systems • Strong communication, organizational, and problem-solving skills • Valid Wisconsin Driver’s License required with an acceptable motor vehicle record (MVR), per FHC guidelines • Bilingual in English and Spanish preferred
