📋 Description

• Process agency home health referrals accurately and timely • Complete data input and client information from referrals • Complete basic diagnosis coding based on referral information • Coordinate care for homecare patients using the Welcome Call Script • Determine referrals from the Clinical Intake team • Interface with agency referral sources • Understand patients’ clinical and service needs • Collaborate with the Intake team to ensure optimal patient care and timely clinical services

🎯 Requirements

• Homecare Homebase (HCHB) experience is required • At least one year of experience in a customer service capacity • Associate’s Degree preferred • At least one year of experience in Home Health preferred • Successful completion of a pre-employment drug screen • Legally authorized to work in the United States
