📋 Description

• Take triage calls from assisted living and memory care sites during nonbusiness hours and weekends • Document directions and necessary information in the EHR for site clinical team follow-up • Prepare activity and incident reports for site clinical teams to analyze call volume and identify staff training needs • Participate in investigations and surveys involving after-hours calls • Promote a positive team approach and healthy work environment • Work the on-call schedule: Monday–Friday 7pm–7am and Saturday–Sunday 7am–7pm and 7pm–7am

🎯 Requirements

• Must reside in Minnesota, Wisconsin, or Colorado • Must maintain a current state license as a Registered Nurse per state regulations • Must be licensed in these states or willing to obtain licensure • Minimum of 5 years’ experience in home health, assisted living, or long-term care preferred • Good organizational skills with ability to self-start and follow through with minimal supervision • Caring heart and passion to care for the elderly • Good judgment, problem-solving, and decision-making skills • Exceptional verbal and written communication skills • Experience using an EHR, specifically Eldermark and PointClickCare • Excellent communication skills, patience, and empathy for onsite staff and residents • Confidence in clinical knowledge and decisiveness • Understanding of pharmacology, basic first aid, and blood glucose monitoring • Past supervisory experience a plus
