📋 Description

• Evaluate patients’/clients’ drug regimen reviews in nursing facilities to assure appropriate drug therapy • Monitor facility compliance with Federal and State guidelines and note irregularities • Provide information regarding medication and Federal and State pharmacy regulations • Act as liaison between the pharmacy and facility to resolve problems • Educate nurses about facility problem areas and staff-interest topics • Attend facility, pharmacy, infection control, quality assurance, and interdisciplinary team meetings • Review physician orders for appropriate action, dosage, potential interactions, administration, and stability • Assist in training and supervising pharmacy technicians, interns, and externs • Produce monthly written reports of findings and recommendations for facility administrators, DONs, and medical directors/physicians • Maintain seizure and drug problem notice files • Reconcile controlled drugs and audit patient charts • Perform other assigned essential functions and duties

🎯 Requirements

• BS/PharmD degree in pharmacy required • Pharmacist License required • Valid driver’s license and clean driving record • 2+ years of related experience, preferably in LTC or high-volume pharmacy • Advanced computer skills; pharmacy operations system experience required • Ability to work independently and deliver to deadlines • Ability to solve problems with minimal direction • Great attention to detail and accuracy • Ability to excel in a fast-paced, team-oriented environment working on multiple tasks simultaneously, while adhering to strict deadlines • Quality minded; motivated to seek out errors and inquire when something appears inaccurate • Requires up to 90% travel
