📋 Description

• Proactively reach out to patients and members through phone, text, chat, email, and other digital channels • Engage patients in healthcare programs, services, and recommended care • Build rapport, identify barriers, and motivate patients to take action • Educate patients about gaps in care, preventive services, screenings, medication adherence, and health-improvement opportunities • Help patients navigate healthcare, schedule appointments, understand next steps, and connect with resources • Manage outbound and inbound interactions while adapting communication to the patient and channel • Use technology and digital tools to communicate, research, document, and manage patient interactions accurately • Collaborate with other teams to resolve patient needs and improve patient experience • Work toward individual and team goals for outreach, engagement, quality, and patient outcomes • Follow security policies and procedures to protect PHI, PII, and company intellectual property

🎯 Requirements

• Experience proactively communicating with customers or patients across phone, text, chat, email, or other digital platforms • Confidence and persistence initiating outbound conversations and building trust • Ability to influence and encourage action through verbal and written communication while remaining empathetic and patient-focused • At least 1 year of experience in healthcare, customer service, contact center, patient/member services, health insurance, pharmacy, medical office, or a related environment • Strong verbal and written communication skills • Familiarity with healthcare terminology, benefits, providers, prescriptions, appointments, or preventive care • Strong computer and problem-solving skills • Ability to learn and work across multiple technologies, systems, and digital communication tools • Ability to work toward measurable goals, apply feedback, and adapt to changing processes and priorities • High school diploma or equivalent • Minimum internet speed of 100 Mbps download and 10 Mbps upload • Ability to provide an internet connection capable of video calls and connecting to internal and external systems • Employees must be accessible during stated working hours • Bilingual language skills or additional healthcare education, training, or certification are a plus
