📋 Description
• Work with one or more APNs performing in-home and/or telehealth wellness assessments on patients designated by HarmonyCares Medical Group • Be available via telecommunication or in person for consultation with APNs regarding patient assessment questions • Regularly perform annual chart reviews • Consult with APNs to ensure adherence to policies, procedures, state laws, and quality standards • Be accountable for outstanding customer service to external and internal customers
🎯 Requirements
• Active/unrestricted Medical Physician License • Board certified/board eligible required • Residency completion required • 1 year experience in Healthcare management • Experience with Risk Adjustment • Must reside in Indiana • Must maintain a valid driver’s license and good driving record
