📋 Description

• Deliver annual risk assessments in residential settings or via telehealth within the nurse practitioner scope of practice, as delegated by the Collaborative Physician • Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and physical, mental, and social needs • Take patient histories, perform examinations, and determine diagnoses • Provide written documentation of patient visits according to NCQA standards • Take patient vital signs as necessary • Place case management referrals and communicate with PCPs as necessary • Communicate with patients, caregivers, agency nurses, other providers, and vendors to assure proper diagnosis • Perform clinical duties while observing OSHA Universal Precautions • Maintain patient confidentiality • Attend required meetings and in-services and participate in committees as requested • Participate in professional development activities and maintain professional licenses and affiliations • Work with teammates, physicians, medical staff, patients, caregivers, agency nurses, providers, and vendors

🎯 Requirements

• Active/unrestricted nurse practitioner license to practice in coverage states • Board certification through ANCC, AANP, or NCCPA • Active BLS Certification • Current enrollment in Medicare/Medicaid • Valid driver’s license and good driving record • Outstanding EHR skills • Ability to adapt to differing weather conditions and patients’ home/residential environments • Full range of body motion, including handling/lifting patients; manual and finger dexterity; eye-hand coordination; normal visual acuity and hearing; standing, bending, walking, and stair climbing • Ability to regularly lift/carry items weighing up to 50 pounds • Ability to ride in an automobile or van up to 150 miles daily in urban and/or rural settings • Ability to drive, if necessary
