📋 Description

• Provide primary care to high acuity patients in their home Monday - Friday • Practice medicine with a focus on patient care, not volume • Take the time needed to truly address patients’ needs • Work within an integrated, team-based, physician-driven model • Use an assistant to support appointments in select markets

🎯 Requirements

• Licensed as a NP in the State where HarmonyCares Medical Group office located • Federal DEA License/eligibility • Active CPR/BLS Certification • Board Certification • Must maintain a valid driver’s license and good driving record • Directly related experience in home health, geriatrics, hospice and primary care • Geriatric training/experience preferred
